You may also print this form and mail it to The Ministerial Seminary of America, LLC, 12154 Darnestown Road, Gaithersburg, MD 20878 along with a check or money order in the amount of $200.00 as your initial payment. Your check or money order will be returned if your application is denied. Balance due will be scheduled as follows:
The balance will be divided into five equal monthly payments.
By completing and submitting this form, student is agreeing to make payments as outlined above. Tuition is nonrefundable. This is a legally binding contract. First payment will be due within 14 days after approval of this application. Students are further acknowledging that Certificates are awarded only upon completion of the course of study in addition to payment of tuition in full.
Once your form is received in the office, you will be contacted as to your approval status and informed as to how to make your first tuition payment, receive your course textbooks, and begin your course of study.
